What is All-on-4?
A full arch of teeth supported by four implants, with the back two tilted to reach solid bone and skip most bone grafting. The bridge is fixed in place. You do not take it out.
How the four-implant design works
The two front implants go in straight. The two back implants are tilted, usually 30 to 45 degrees, so their tips reach the dense bone at the back of the jaw instead of the thin bone under the sinuses. That tilt is the whole idea: longer implants, better grip, and a bridge supported along the full arch rather than just the front. At Smile Now, Dr. Broyde also uses the pterygoid bone behind the upper jaw whenever it is available, which is why many of our All-on-4 arches end up with five or six implants.

Fixed, not removable
The bridge is screwed to the implants. Nothing comes out at night and nothing shifts when you bite into an apple.
Usually no bone graft
Tilted and pterygoid implants use the bone you already have. Patients told elsewhere that they lack bone are often treated without grafting.
More than four when it helps
Four is the minimum, not the target. Where the bone allows, Dr. Broyde adds a fifth or sixth implant so the back of the bridge is supported rather than cantilevered.
Zirconia finals
Acrylic bridges stain, chip and wear. Your final arch is zirconia, which holds its colour and shape for years.

Consultation and 3D scan
Duration: 60-90 minutes
A cone-beam CT scan shows us how much bone you have and where. Dr. Broyde reads it with you in the room and tells you whether four implants will do or whether a fifth or sixth makes sense. You leave with a written flat-fee quote, not a range.

Surgery day
Duration: 2-3 hours per arch
Under IV sedation, any failing teeth come out and the implants go in during the same sitting. Dr. Broyde works from the scan and the feel of the bone, placing the back implants on an angle, or into the pterygoid bone on the upper jaw, so each one has solid grip.

Temporary teeth before you leave
Duration: 1-2 hours
While you rest, the in-house lab makes your temporary bridge from the morning's records. It is screwed onto the implants that afternoon. You go home with fixed teeth, a soft-food list and aftercare instructions.

Healing and the zirconia final
Timeline: Final restoration at 4-6 months | Warranty: 5 years
Over about four months the implants fuse with the bone. We then take new impressions and mill your final zirconia bridge in our lab. The temporary comes off, the final goes on, and the warranty starts.

Neither option is better in the abstract — they answer different problems. Traditional individual implants remain the right choice when you're replacing one to three teeth and have healthy bone around them. All-on-4 exists for the case where an entire arch is failing or already gone, and it wins there on the two things that actually decide it for most patients: you leave with fixed teeth on the day of surgery, and the angled placement usually means no bone graft and no extra healing period before treatment can start.
